Selection of K65R mutation in HIV-2-infected patients receiving tenofovir-containing regimen.

چکیده
Studies based on selective pressure imposed by tenofovir in vitro [1] have reported the emergence of the K65R mutation in HIV-1 reverse transcriptase (RT) gene. The K65R mutation is known in HIV-1 to confer reduced antiviral activity to tenofovir, abacavir and lamivudine and retain activity to thymidine analogues [2]. Few data are available on the selection of resistance mutations in HIV-2-treated patients and none in patients receiving tenofovir. We studied which substitutions are selected in HIV-2-infected patients included in the French ANRS cohort receiving tenofovircontaining regimen.

Decreasing population selection rates of resistance mutation K65R over time in HIV-1 patients receiving combination therapy including tenofovir.
OBJECTIVES The use of tenofovir is highly associated with the emergence of mutation K65R, which confers broad resistance to nucleoside/nucleotide analogue reverse transcriptase inhibitors (NRTIs), especially when tenofovir is combined with other NRTIs also selecting for K65R. Although recent HIV-1 treatment guidelines discouraging these combinations resulted in reduced K65R selection with tenof...
متن کاملK65R in Subtype C HIV-1 Isolates from Patients Failing on a First-Line Regimen Including d4T or AZT: Comparison of Sanger and UDP Sequencing Data
BACKGROUND We and others have shown that subtype C HIV-1 isolates from patients failing on a regimen containing stavudine (d4T) or zidovudine (AZT) exhibit thymidine-associated mutations (TAMs) and K65R which can impair the efficacy of Tenofovir (TDF) at second line. Depending on the various studies, the prevalence of K65R substitution as determined by the Sanger method ranges from 4 to 30%. Ou...
متن کاملHigh Prevalence of the K65R Mutation in HIV-1 Subtype C Infected Patients Failing Tenofovir-Based First-Line Regimens in South Africa
BACKGROUND Tenofovir (TDF) has replaced stavudine (d4T) as the preferred nucleoside reverse transcriptase inhibitor (NRTI) in first-line regimens in South Africa, but limited information is available on the resistance patterns that develop after the introduction of TDF. This study investigated the antiretroviral drug resistance patterns in South African HIV-1 subtype C-infected patients failing...
متن کاملFactors associated with the emergence of K65R in patients with HIV-1 infection treated with combination antiretroviral therapy containing tenofovir.
BACKGROUND The human immunodeficiency virus type 1 reverse-transcriptase mutation K65R is a single-point mutation that has become more frequent after increased use of tenofovir disoproxil fumarate (TDF). We aimed to identify predictors for the emergence of K65R, using clinical data and genotypic resistance tests from the Swiss HIV Cohort Study. METHODS A total of 222 patients with genotypic r...
متن کاملClonal resistance analyses of HIV type-1 after failure of therapy with didanosine, lamivudine and tenofovir.
BACKGROUND The rapid failure of initial therapy with combinations of nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs) that exclude zidovudine has not been fully explained by standard virus population analyses of HIV type-1 (HIV-1) drug resistance. We therefore investigated HIV-1 genotype and phenotype at the single genome level in samples from patients on a failing regimen of teno...
